Description Translational research aims at converting the molecular knowledge generated by preclinical research into applications targeted at improving diagnosis and treatment. In Oncology, this is leading to genetic therapies based on the background of the individual patient and on the molecular profile of the individual tumor. The so called "Personalized" Therapy represents the full realization of this new approach, which is based on the assumption that the therapeutic response to a targeted drug depends on the presence and role of the target in the tumor which needs to be treated. In particular, those cancers in which the target is constitutively activated should respond to the corresponding molecular therapy. In the future, the ability to diagnose and successfully treat patients with cancer will be closely dependent on two main factors: (a) the thorough understanding of the molecular determinants of cancer and (b) the ability to translate this information into new diagnostic tests to give each patient the most suitable drug. In this scenario, the identification of new "predictive biomarkers", that is molecular characteristics capable of addressing the use of a particular drug in a specific population of patients, becomes a factor of considerable importance. To date, clinical and empirical parameters are mainly used for the design of clinical trials, with a limited number of clinical studies using molecular markers for the selection of patients to be treated. Despite obvious scientific, ethical, social and economic reasons, the early introduction of these markers in clinical trials is faltering. NMS has several clinical trials on new targeted therapies currently ongoing and is certainly interested to increase its knowledge in this area. This project proposes the ambitious goal of creating a multidisciplinary Centre able to tackle and develop the main themes of the translational research applied to cancer therapy. The project intends to trigger in Sicily the development and application of frontier technologies in genetics, genomics, bioinformatics and proteomics for the molecular characterization of various cancers, with particular regard to: (1) creating a large panel of cell lines characterized under their genetic genomic and proteomic profile, associated with the use of automated technologies for their pharmacological treatment and the identification of drug sensitivity biomarkers (2) the application of systems biology and bioinformatics approaches for large scale data analysis and integration of knowledge in mathematical models that facilitate their interpretation; (3) the creation of a bank of human tumor biopsies, prepared and preserved appropriately for molecular characterization. The overall objectives are two: 1) identifying new predictive biomarkers of sensitivity to drugs under development at NMS; (2) the development of procedures for the analysis of at least 3 biomarkers, already in use in research and/or clinical practice in Oncology. The associated training project aims to train researchers in the field of genomics, proteomics and bioinformatics, in order to develop high profile competencies in these areas. Thanks also to the localization at IOM research and contiguity with other cancer research laboratories, this project aims at creating a network based on operational protocols validated and shared between public and private centers in Sicily, but certainly open to other partnerships in Italy and abroad. The project is based on the scientific excellence and the complementary nature of the participants, including hospitals, research institutions, large enterprises and SMES, converging towards achieving objectives with significant repercussions for employment and re-launching of the local territory, and an overall strategic importance of international level.
