Description Diabetes mellitus represents one the leading causes of mortality in the contemporary society. It could be estimated that in the next decade, type 2 diabetes will become the most relevant chronic illness to be considered by the National and International Health Care Systems worldwide, including Italy. Although most frequently diagnosed in the adults, type 2 diabetes incidence is rapidly growing in the adolescents. Intensive programs targeted at altering lifestyle risk factors for type 2 diabetes have demonstrated substantial efficacy in reducing diabetes incidence in at-risk individuals. However, the rapid and widespread implementation of lifestyle interventions to reduce diabetes also proved to be difficult to accomplish outside of the clinical trial, further underlying the importance of improving the effectiveness and the precision of treatment and prevention through better understanding of the genetic and molecular aetiology of the disease. While genetics is often viewed as a tool to aid drug discovery, perhaps more immediately applicable is the notion that the presence of particular genetic variants will influence individual response to therapy. Thus, identifying T2D-associated genetic variation is expected to guide better drug use in the clinic. The long-term objective of the present program is the development of diagnostic tools based upon geno- and phenotypical characterization, as well as transcrptomic analysis, of T2D to differentiate patient subgroups in order to tailor therapeutic approaches to the disease. The project aims are the following: 1) To generate a data base with information on the onset and the progression of type 2 diabetes and (including the onset of diabetic complications and drug-resistance); 2) To replicate in the population of Regione Campania the association of the most common allelic variants of specific genes with the risk of developing type 2 diabetes; 3) To analyze the association of known SNPs of known genes with the presence of resistance to specific drugs in the common forms of type 2 diabetes by “pyrosequencing” procedures; 4) To identify novel molecular pathways, or novel molecura targets within known pathways in diabetic patients who developed drug-resistance, by “whole transcriptome” analysis using a new generation sequencing platform (SOLiD); 5) To generate a diagnostic tool for the identification of allelic variants of genes associated to drug-resistance in the common forms of type 2 diabetes.
